Access Your Forma Card

From your profile at the top, click on Settings, then click on Cards.

When you use the Forma Card, no reimbursement is required.
You can request a physical card within the Forma portal.
If you use the Forma Card for a taxable wallet (Food, Wallet, or S.A.F.E.), you will be taxed via payroll.

Access the Forma Store

From the navigation bar, click on Store.

Enjoy up to 30% off retail.

Wallet Options

Starting January 1st, 2024, the S.A.F.E Journey wallet will be limited to adoption, surrogacy, and long-term egg, sperm, and embryo storage expenses. This wallet will remain a $20k lifetime maximum and will continue to be taxed.

IMPORTANT: Fertility services have moved to Cigna plans, starting on January 1st, 2024.
