Affirm offers multiple medical plan options to fit your needs. All medical plans provide in-network preventive
care at no cost to you.

Which plan is right for you?

Do you prefer specific doctors or hospitals?

If you want to stay with your preferred doctors and facilities, check whether they are in the planโ€™s network. If they are not, but you are comfortable paying a bit more to see them, consider a plan with both in-network and out-of-network coverage.

What are your usual healthcare needs?

How often do you anticipate seeking care services? Do you have a condition that requires a specialist? Do you take prescription medications? Compare how each plan covers the services you need most often.

Consider the bottom line.

Do you have to meet a deductible? What is the out-of-pocket maximum? How much of the cost is covered by the plan? How much are any copayments for office visits, prescriptions, etc. All of these factors together affect your total cost of healthcare.

What are my medical plan options?

Cigna OAP

The Cigna OAP medical plan features copays for common services such as primary care or specialty visits with no deductible required. For other services such as eligible inpatient and outpatient care, youโ€™ll pay coinsurance after you satisfy your deductible up to the out-of-pocket maximum. This plan also includes transgender health services.

 

Cigna OAPIN

The Cigna OAPIN medical plan features copays for most services with no deductible to satisfy before the plan pays for eligible services. The OAPIN does not include out-of-network coverage.

 

Kaiser HMO

The Kaiser HMO medical plan features copays for most services with no deductible to satisfy before the plan pays for eligible services. The HMO does not include out-of-network coverage, so youโ€™ll need to coordinate your care with a Primary Care Physician (PCP).

Kaiser is only available to California residents.
